Special NICs Issued for People with
Disabilities 

What is the procedure to get
disability certificate 

The
steps through which I obtained this particular card (Special CNIC)
are as follows. 

Step 1 

In
the first step, you will appear in front of the medical board (medical superintendent
MS). When you visit the relevant hospital (DHQ) to make your medical check-up,
the hospital staff will provide you with information about the day and date of
the medical check-up. 

When you visit the hospital on the day of the inspection,
bring your X-ray and doctor’s prescription with you. Remember, the medical
inspection form is available in the Photostat shop. 

(I request to the
hospital’s concerned authorities that the medical check-up form should be
available within the hospital). Persons with disabilities face a major
problem in obtaining this form.) 

Step 2 

When
you have completed your medical examination, at that point you will visit the
Polytechnic and Vocational Colleges in the second step after obtaining a
certified medical form. Once you have verified the forms from these two related
colleges then. 

Step 3 

In
the third step, you will visit the Office of Employment Exchange Program. There
you will need to submit these required forms, (i.e., medical forms and
certified forms from colleges to this office), in the Office of Employment
Exchange Program. 

Step 4 

After that you
will visit the Social Welfare Office in the fourth step. Submit all these
certified forms out there. The rest of the procedure will explain to you the
relevant staff of the Social Welfare Office. 

Last Step 

After
completing all these steps, all the certified forms should be taken to the
nearest NADRA office. The rest of the procedure will explain to you the
relevant staff of the NADRA office. 

There is no fee for making this special
card. The Government of Pakistan will provide this card to you in free. Please
inform your NADRA staff officer that I need this special disability card
in the form of a SMART CARD. 


Note: You will also need a four-digit passport-sized image and Prepare
four photocopies of your simple computerized ID card in advance to carry out
this method. Therefore, manage them as well. Because when you go to the social
welfare office in the last step, you will paste these pictures in the social
welfare form there. A 17-grade government officer (manager of National bank
only NOT Private bank managers) should attest to the NADRA application form.

this Special CNIC in a week or in few days. The wheelchair logo will be made on this
disabled person’s card).

مراحل سے میں نے یہ مخصوص کارڈ حاصل کیا  وہ
درج  ذیل ہیں۔

یہ خصوصی کارڈ بنانے کے لیے کوئی فیس نہیں ہے۔


مرحلہ
نمبر ۱:

پہلے
مرحلے میں ، آپ میڈیکل بورڈ (میڈیکل سپرنٹنڈنٹ ایم، ایس) کے سامنے پیش ہوں گے۔ جب
آپ اپنا میڈیکل چیک اپ)  کرنے کے لیے جا ۓگے ،  متعلقہ ہسپتال  کا عملہ آپ کو میڈیکل چیک اپ کے دن اور تاریخ کے
بارے میں معلومات فراہم کرے گا۔ جب آپ معائنہ کے دن ہسپتال جائیں تو اپنا ایکسرے
اور ڈاکٹر کا نسخہ اپنے ساتھ لائیں۔ یاد رکھیں ، طبی معائنہ فارم فوٹو سٹیٹ شاپ میں
دستیاب ہے۔ (میں ہسپتال کے متعلقہ حکام سے درخواست کرتا ہوں کہ میڈیکل چیک اپ فارم
ہسپتال کے اندر دستیاب ہونا چاہیے)۔ معذور افراد کو یہ فارم حاصل کرنے میں ایک بڑی
پریشانی کا سامنا کرنا پڑتا ہے۔

مرحلہ
 ۲ :

جب
آپ اپنا طبی معائنہ مکمل کر لیں گے ، اس وقت آپ دوسرے مرحلے میں پولی ٹیکنک اور
ووکیشنل کالجوں کا دورہ کریں گے۔ جب آپ نے ان دو متعلقہ کالجوں سے فارموں کی تصدیق
کر لیں تو اس کے بعد آپ۔

مرحلہ
۳:

تیسرے
مرحلے میں ، آپ آفس آف ایمپلائمنٹ ایکسچینج پروگرام جاۓ گے۔ وہاں آپ کو یہ مطلوبہ
فارم ، (یعنی کالجوں سے تصدیق شدہ فارمز،  میڈیکل فارم اور مصدقہ فارم اس آفس میں) ، آفس
آف ایمپلائمنٹ ایکسچینج پروگرام میں جمع کرانے ہوں گے۔

مرحلہ
۴:

اس
کے بعد آپ چوتھے مرحلے میں سوشل ویلفیئر آفس جائیں گے۔ یہ تمام مصدقہ فارم وہاں جمع
کروائیں۔ باقی طریقہ کار آپ کو سوشل ویلفیئر آفس کا متعلقہ عملہ سمجھائے گا۔

آخری
مرحلہ:

ان
تمام مراحل کو مکمل کرنے کے بعد تمام مصدقہ فارم قریبی نادرا آفس میں لے جانے چاہئیں۔
باقی طریقہ کار آپ کو نادرا آفس کا متعلقہ عملہ سمجھائے گا۔ یہ خصوصی کارڈ بنانے
کے لیے کوئی فیس نہیں ہے۔ حکومت پاکستان یہ کارڈ آپ کو مفت میں فراہم کرے گی۔
برائے مہربانی اپنے نادرا سٹاف آفیسر کو مطلع کریں کہ مجھے یہ خصوصی معذوری کارڈ اسمارٹ
کارڈ کی شکل میں مہیا کیا جاۓ۔

نوٹ:
آپ کو چار عدد پاسپورٹ سائز کی تصویر کی بھی ضرورت ہوگی اور اس طریقہ کار کو انجام
دینے کے لیے اپنے سادہ کمپیوٹرائزڈ شناختی کارڈ کی چار فوٹو کاپیاں پہلے سے تیار
کریں۔ لہذا ، ان کا بھی انتظام کریں۔ کیونکہ جب آپ آخری مرحلے میں سماجی بہبود کے
دفتر جائیں گے، تو آپ ان تصاویر کو وہاں سماجی بہبود کے فارم میں چسپاں کر دیں گے۔
17 گریڈ کے سرکاری افسر (صرف نیشنل بینک کے منیجر) کو نادرا کے درخواست فارم کی
تصدیق کرنی چاہیے۔

سرٹیفیکیشن حاصل کرنے کے لیے ضروریات:

 

چار پاسپورٹ سائز کی تصویر۔

قومی شناختی کارڈ یا  فارم ‘ب’  کی چار کاپیاں (درخواست گزار کے لیے 18 سال سے
کم عمر کی صورت میں)

1/پیدائش کا سرٹیفکیٹ کی کاپی۔

1/میٹرک سرٹیفکیٹ کی کاپی۔

  نوٹ:  (آپ کو یہ
خصوصی کارڈ ایک ہفتے یا چند دنوں میں موصول ہو جائے گا۔ اس  خصوصی کارڈ پر وہیل چیئر کا  لوگو بنا ہو گا)۔
